Quilt Registration: There are two methods available to register a quilt  

Either one works, only choose one!

 

1. The first are FILLABLE FORMS. Use these forms if you want to type your information into the form online.  Then save the completed form(s) to your computer and email them to Janet Denson.  You will need to still USPS mail your check, and photos (or deliver in person at an OWQG meeting).

  2. The second are PRINTABLE FORMS. Use these forms if you want to print blank forms, then

     fill in with pen or pencil.  When complete, USPS mail forms, check, and photos (or deliver in person at

     an OWQG meeting).

Whichever method you use, you MUST read and follow all directions, which includes sending your Quilt Display Card to Janet Denson at janetdenson@comcast.net or
